Clinical Effects of Pneumatic Retinopexy for Persistent Retinal Detachment after Scleral Buckling Surgery

Abstract
Purpose: The author evaluated the effects and problems of pneumatic retinopexy (PR) for persistent retinal detachment after scleral buckling surgery.

Methods: A retrospective chart review was done for patients who underwent PR for persistent retinal detachment due to persistent opening of retinal tears after scleral buckling surgery. The author analysed the reasons for failure of retinal reattachment, and described the fundus findings of these patients.

Results: PR failed to achieve permanent retinal reattachment in 9 of the 20 eyes. Of these 9 eyes, retinal redetachment occurred after PR by the development of a new retinal tear in 7 eyes, and by the persistent opening of an original retinal tear with continuous residual vitreous traction in 2 eyes. And lattice degenerations were observed more frequently in the eyes that developed new retinal tears after PR.

Conclusions: PR can cause new retinal tears to occur in the eyes with vitreoretinal adhesions such as lattice degenerations. Therefore, in the cases of failed scleral bucking surgery, number of reoperations can be reduced and early retinal reattachment can be achieved by careful patient selections and appropriate surgical procedures.
